Grantee teams of five will receive in-depth, face-to-face training on the strategic implementation of Practice-Based Coaching strategies to support the transfer of knowledge from training into practice. This training will include guided development of individualized action planning for local implementation and continued success.

•The Three Ps of Programmatic Support
•Team Planning – The PATH Process
• Closing

NOTE: Lunch will be provided on day 1 & 2.

Grantee teams of 5 participants preferred.
Minimum of 3 team members per grantee team are required.
